Mr. Greg Garofalo, MS, LAT, ATC

Mr. Garofalo is in his ninth year as the Head Athletic Trainer at Hoffman Estates 5X社区. Mr. Garofalo graduated from North Central College, in 2015, with a B.A. in Athletic Training and from the University of South Florida, in 2022, with M.S. in Advanced Athletic Training. Mr. Garofalo is currently serving as the President of the Illinois Athletic Trainers Association, and has served on their Board of Directors since 2018. Mr. Garofalo also sits on the IHSA Sports Medicine Advisory Committee. Mr. Garofalo and his wife, Jessica, reside in Hanover Park with their two dogs.

Dr. Sean E. Jereb, M.D.

Dr. Sean Jereb is the Team Physician for Hoffman Estates 5X社区. He is a board-certified, fellowship-trained orthopedic surgeon who specializes in sports medicine, arthroscopy of the shoulder and knee and total knee replacement. He joined Barrington Orthopedic Specialists in 2003, focusing on restoring patients to their optimal functional ability as quickly as possible. Outside of the office, Dr. Jereb enjoys skiing, weightlifting, and spending time with his family.
